What We Do

Personalized support

Diabetes Partnership of Cleveland has diabetes educators on staff to provide personalized support and education to children and adults with pre-diabetes and diabetes.  Many have no or limited health insurance and, therefore, have limited access to affordable diabetes education. It requires intensive self-management to stave off devastating and potentially lethal complications.  Uncontrolled diabetes is the leading cause of blindness, end-stage kidney disease and below the knee non-traumatic caused amputations in adults.  These complications carry shattering physical, emotional and financial burdens for individuals, their families, the workforce and the health care system. 

Our goal is to instill self-efficacy in our clients so they can learn how to take care of themselves, change the course of their disease and improve their lives.  We help clients focus on taking charge of their disease through participation in group discussions, developing relationships that provide peer support, learning appropriate problem-solving techniques, setting realistic goals through personal action plans and acquiring coping skills.  Because we incorporate successful research-based practices in all our programs, our process almost always increases the motivation of our clients to manage their disease.

Direct services & Community outreach

Our services include: individual and group diabetes self-management education, community outreach ; telephone counseling and referral; benefits counseling; diabetes risk assessment including glucose finger stick; peer support groups; assistance with blood glucose monitoring supplies,insulin syringes and an emergency insulin program as well as Camp Ho Mita Koda, a residential camp for children with diabetes.  Our services complement, but do not replace, medical care.  We also provide a venue for peer support and reinforcement of sustaining long-term behavioral changes.
